DISCLOSURE OF INFORMATION BY INSURERS
(applies only to small group plans (1-50 employees)

When an insurer offers to sell insurance to a small group plan or upon request, insurers must provide information regarding:

the insurer's right to change rates and the factors affecting those rates

guaranteed renewability

any pre-existing condition exclusions including use of the alternative method of counting creditable coverage

any affiliation periods applied by HMO's

the geographic areas served by HMO's

benefits and premiums available under all health insurance coverage for which the employer is qualified
